Hi, my name is Natasha and I am the Lead Clinical Pharmacist for the Viaduct Primary Care Network.
About My Role
I have been working in Primary Care for almost 6 years and joined our Primary Care Network (PCN) two and a half years ago. Some of my previous roles include Medicines Optimisation Pharmacist and Pharmacy Manager.
During my time at the PCN I have qualified as an independent prescriber and completed the CPPE Primary Care Pathway to further develop my knowledge and expertise.
My role as the Lead Clinical Pharmacist includes mentoring and supporting my team to ensure we deliver excellent patient care. I also provide clinical support and work closely with the Clinical Director and Manager to strategically achieve wider PCN goals.
Working within practice my role includes issuing medication, completing medication reviews, reconciling clinic letters and hospital discharge summaries. I work within a multidisciplinary team and use my expertise to resolve medication queries from the team and from patients.
