Our Pharmacy Technicians ensure effective medicines use, through shared-decision making conversations with patients followed by supporting medication reviews.
Our Advanced Clinical Practitioners improve clinical continuity and provide more patient-focused care. In addition they also help provide safe, accessible, and high-quality care.
Founded in 2019 Viaduct has grown into a well respected primary care network covering 8 practices in the Huddersfield area. We aim to increase the services offered to our patients to enhance their health and improve the care they receive.
As a primary care network our main objective is to build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home.
